Witnessing both Dean's resilience against torture and the horrific last ten years of his brother's life, Sam is violently dispossessed of his notion that Dean is weak. In fact, since Dean wasn't there to soften the revelation, you could say Sam's a bit scared of his brother now...
Go ahead and make Dean creepier than in canon to up Sam's freakout factor if you like. I'm as fond of scary, torturer!Dean as I am of people getting freaked out by misunderstandings.
If you'd like to focus on one aspect to the exclusion of the other, go ahead!

The smile on Veronica's face faltered as she remembered waking up to the sun shining over her and the warmth of a body pressed against her own. Even though that morning felt like a lifetime ago, Veronica could still hear Lilly's giggles and could still remember the feeling of their lips pressed together.
Veronica looked down at the cup in her hand, briefly thinking about taking a drink instead of answering, but then she remembered Lilly's warm smile and the soft, "I love you, Veronica," whispered to her after they kissed.
After a long moment, Veronica looked up and said, "Her name was Lilly, and she was my best friend."

Faces float behind his eyelids, flashes of dreams that vanish almost as quickly as they appear. Then, they recede back into the blackness that he has so long called home. From somewhere deep in his cotton-stuffed throat comes a low grunt, a mere idea of a word. It barely reaches his own ears before the blizzard encasing him swallows it whole.
