Witnessing both Dean's resilience against torture and the horrific last ten years of his brother's life, Sam is violently dispossessed of his notion that Dean is weak. In fact, since Dean wasn't there to soften the revelation, you could say Sam's a bit scared of his brother now...
Go ahead and make Dean creepier than in canon to up Sam's freakout factor if you like. I'm as fond of scary, torturer!Dean as I am of people getting freaked out by misunderstandings.
If you'd like to focus on one aspect to the exclusion of the other, go ahead!

Upon its rebirth, the Viesczy may be identified by its habit of biting itself. The Viesczy's hunger is great, but the threat it poses is distinctly revenant-like in nature: It thirsts only for the blood of its family.

Even though that morning felt like a lifetime ago, Veronica could still hear Lilly's giggles and could still remember the feeling of their lips pressed together.
Veronica looked down at the cup in her hand, briefly thinking about taking a drink instead of answering, but then she remembered Lilly's warm smile and the soft, "I love you, Veronica," whispered to her after they kissed.
After a long moment, Veronica looked up and said, "Her name was Lilly, and she was my best friend."
